29 CFR 1926.451(b)(5)(ii): Each platform greater than 10 feet in length shall not extend over its support more than 18 inches (46 cm), unless it is designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.    a. On or about January 29, 2013  - at the above addressed jobsite, employees were performing siding installation from a ladder jack scaffold that had the platforms extending greater than eighteen (>18") inches past its supports, thereby exposing the employees to fall hazards of approximately twelve (12) feet six (6) inches.

On or about January 29, 2013 - at the above addressed jobsite, employees were performing siding installation from a ladder jack scaffold approximately twelve (12) feet six (6) inches above the ground.  The scaffold platforms were bridged one to another, thereby exposing the employees to the hazards associated with falls.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    a.  On or about January 29, 2013 - at the above addressed jobsite, employees conducting siding installation from a ladder jack scaffold approximately twelve (12) feet six (6) inches above the ground were not provided with an access ladder, thereby exposing the employees to fall hazards.

The training shall include the following areas, as applicable:    a.  On or about January 29,, 2013 - at the above addressed jobsite, employees using the ladder jack scaffold were not trained to recognize the hazards associated with the use of such scaffolds, thereby exposing the employees to the hazards associated with falls.
