29 CFR 1926.451(b)(4): Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15 cm):    a) 25 Blackhorse Court, Staten Island, NY - Employee was performing Stucco finishing work while standing on a board traversed from a 12-foot wooden Husky ladder and a 6 foot Werner ladder, that was not extend past the wooden ladder and/or was not secured to the ladder.    Condition observed on or about January 29, 2013.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, stucture, personnel hoist, or similar surface was not used.  Cross-braces were used as a means of access:    a) 25 Blackhorse Court, Staten Island, NY -Employees were exposed to fall hazards of approximately 13' 1" to the ground below when a ladder was not provided for safe access to a scaffold platform.    Condition observed on or about January 29, 2013.
