29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.    At the jobsite located at 9692 Village Place, Maize KS:  An employee engaged in material handling activities, for the purpose of loading decking material onto the roof, was exposed to struck-by hazards in that head protection was not provided.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.    At the jobsite located at 9692 Village Place, Maize KS:  Employees engaged in decking installation activities on a 4:12 pitch roof were exposed to fall hazards of approximately 9 feet from the eave to the ground below in that fall protection was not provided.
