Asbestos Abatement Project Monitoring Report, dated October 18, 2011, did not include information about the asbestos-containing thermal system insulation covering the overhead pipes in the sub-basement located in Building E.  The asbestos containing pipe insulation was in a friable and damaged condition.  Bulk sampling revealed the presence of 30% Chrysotile asbestos. The employer did not attach such labels in areas where they will clearly be noticed by employees who are likely to be exposed, such as at the entrance to mechanical room/areas. Signs required by paragraph (j) of this section may be posted in lieu of labels so long as they contain the information required for labeling.    a. Maintenance employees worked in the vicinity of previously installed asbestos containing pipe insulation.
