29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    a)  At 1924 West Beach Blvd Gulf Shores, Alabama:  On or about January 29, 2013 and at times prior; the employer exposed his employees to fall hazards of 11.1 feet to the ground below in that he failed to provide fall protection for employees working from a ladder jack scaffold.    b)  At 1924 West Beach Blvd Gulf Shores, Alabama:  On or about January 29, 2013 and at times prior; the employer exposed his employees to fall hazards of 25.3 feet to the ground below in that he failed to provide fall protection for employees working from a ladder jack scaffold.

29 CFR 1926.452(k)(4): Ladders used to support ladder jacks shall be placed, fastened, or equipped with devices to prevent slipping:    a)  At 1924 West Beach Blvd Gulf Shores, Alabama:  On or about January 29, 2013 and at times prior; the employer exposed his employees to fall hazards of 25.3 feet to the ground below in that he failed to ensure ladders used to support the ladder jack scaffold were secured to prevent the scaffold from slipping and collapsing.    b)  At 1924 West Beach Blvd Gulf Shores, Alabama:  On or about January 29, 2013 and at times prior; the employer exposed his employees to fall hazards of 11.1 feet to the ground below in that he failed to ensure ladders used to support the ladder jack scaffold were secured to prevent the scaffold from slipping and collapsing.
