29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents. On or about 1/28/13, an employee of Premier Culture Stone, Inc. was observed saw cutting brick at a building undergoing construction located at the northeast corner of North Oak and West State in O'Fallon, IL. The employee was exposed to eye injury from concrete dust as he was not wearing eye protection during the process.

29 CFR 1910.134(a)(2): A respirator shall be provided to each employee when such equipment is necessary to protect the health of such employee. The employer shall provide the respirators which are applicable and suitable for the purpose intended. The employer shall be responsible for the establishment and maintenance of a respiratory protection program, which shall include the requirements outlined in paragraph (c) of this section. The program shall cover each employee required by this section to use a respirator. On or about 1/28/13, an employee of Premier Culture Stone, Inc. was observed saw cutting brick at a building undergoing construction located at the northeast corner of North Oak and West State in O'Fallon, IL. The employee was exposed to potential respiratory irritation and/or scarring, and the employer had not performed a hazard assessment for the work, nor was respiratory protection provided.
