29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Injection Molding Shop: The employer failed to develop written procedures to control the hazardous energy for the six injection molding machines located in the shop area. These procedures need to include the shut down and re-energizing of all energy sources including electrical, hydraulic and thermal energy. Employees were exposed to amputations, burns and electrocution from the unexpected release of energy while performing maintenance and set up of the injection molding machines.

Employees would be exposed to the release of uncontrolled hazardous energy if the procedures are not adequate.
