29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:    a) Jobsite, Washington Ave. Bridge, southeast side, on or about 1/30/13 -- Existing slope had been scaled back to allow for bridge pier replacement; slope of scaled area exceeded maximum allowable slope for type C soil, exposing employees to possible collapse hazard.
