Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about 1/30/2013, 755 E. 233rd Street    a) The employee was working on an unprotected roof, which measured 7' wide, 15' long and approximately 13' high. He worked from an a-frame ladder to weld brackets. He was exposed to an 18' fall hazard.
