29 CFR 1904.31(b)(1): The employer failed to correctly complete the OSHA 300 Form, Log of Work-Related Injuries and Illnesses, regarding covered employees:  (a) For calendar year 2009, where the employer over recorded approximately five entries on the OSHA 300 Log where the injured party was a self-employed contractor at 1600 East Benson Road, Sioux Falls, South Dakota.    (b) For calendar year 2010, where the employer over recorded approximately three entries on the OSHA 300 Log where the injured party was a self-employed contractor at 1600 East Benson Road, Sioux Falls, South Dakota.    (c) For calendar year 2011, where the employer over recorded approximately three entries on the OSHA 300 Log where the injured party was a self-employed contractor at 1600 East Benson Road, Sioux Falls, South Dakota.
