29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation: In the following instances supported scaffold legs did not bear on adequate firm foundations: a) Southwest Scaffold - On or about 1/29/13, three scaffold legs did not bear on firm foundations. b) Southeast Scaffold - On or about 1/29/13, four scaffold legs did not bear on firm foundations. c) Northeast Scaffold - On or about 1/29/13, two scaffold legs did not bear on firm foundations. d) Southern Porch - On or about 1/10/13, an employee, utilizing a fabricated frame scaffold, fell when at least one of the scaffold legs broke through a sheet of OSB.

29 CFR 1926.452(c)(2): Frames and panels shall be braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally. The cross braces shall be of such length as will automatically square and align vertical members so that the erected scaffold is always plumb, level, and square. All brace connections shall be secured. In the following instances fabricated frame scaffolds were not adequately braced: a) Southwest Scaffold - Four cross braces were missing. b) Southeast Scaffold - Eight cross braces were missing. c) Northeast Scaffold - Four cross braces were missing. d) West Scaffold - Five cross braces were missing.

29 CFR 1926.452(j)(2): Poles shall be secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. When the pump jack has to pass bracing already installed, an additional brace shall be installed approximately 4 feet (1.2 m) above the brace to be passed, and shall be left in place until the pump jack has been moved and the original brace reinstalled. East Scaffold - The two poles on the pump jack scaffold were not equipped with bottom braces.
