On or about February 7, 2013 The employer failed to ensure that proper procedures were followed when an employee opened a work panel to adjust a condenser drum on the Doctor Otto Anglightner (DOA) machine while it was energized. The employee was exposed to the machine's rotating parts while conducting work that could result in caught-in and nip-point injuries.
