29 CFR 1926.405(g)(2)(iv):     Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.    a)	On or about January 1, 2013; at the center of the jobsite, the employer allowed employees to utilize an extension cord with inadequate strain relief in order to power two (2) hand-held drills.  Employees used the hand-held drills to install ceiling channel.

Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification.    a)	On or about January 1, 2013; at the center of the jobsite, the employer allowed employees to utilize extension cords in a manner that was not in accordance with instructions included in the labeling.  Employees connected extension cords together to create a chain of three (3) extension cords and one (1) multi-tap cord in order to power two (2) hand-held drills.  Employees used the hand-held drills to install ceiling channel.
