29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a) On or about January 1, 2013; at the center of the jobsite, the employer allowed employees to utilize an extension cord that was missing a ground pin. The extension cord was used to deliver power from an outlet box to a series of extension cords, which were used to energize two (2) hand-held drills. Employees used the hand-held drills to install ceiling channel. The missing ground pin prevented the extension cord from having a continuous path to ground.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a) On or about January 1, 2013; at the center of the jobsite, the employer allowed employees to utilize an extension cord with inadequate strain relief in order to power two (2) hand-held drills. Employees used the hand-held drills to install ceiling channel.

Employees connected extension cords together to create a chain of three (3) extension cords and one (1) multi-tap cord in order to power two (2) hand-held drills.
