At the worksite located at 501 West Shrock Road in Westerville, Ohio the employer had a maintenance employee that assisted in the changing of a fuse on a 480 VAC 3 phase switch which resulted in an arc blast that sent the employee to the hospital, and the employer did not provide effective training to their employee that would give him the skills necessary to recognize and control the hazards with that task.
