29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Two employees dismantling a fabricated frame scaffold were exposed to struck by hazards due to the collapse of the scaffold and the employer did not provided training from a person qualified in the recognition of the hazards and the procedures to control such hazards while working with a fabricated frame scaffold.
