29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about 02/01/13 at Conyers, GA: Employees were exposed to caught between hazards while operating a South Bend lathe at their maintenance shop.  The employer failed to adequately guard the lathe's point of operation (chuck).    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a power industrial truck:     On or about 02/01/13 at Conyers, GA: Employees were exposed to struck-by hazards while operating a Nissan forklift at the maintenance shop.  The employer failed to implement a comprehensive forklift training program, which included operator training on all required elements as required by the standard.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
