29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems or personal fall arrest system:  (a)  On or about 02/02/13 - 705 Martin Street North, Pell City, AL, employees working at the edge of a roof were exposed to falling 12 feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a)  On or about 02/02/13 - 705 Martin Street North, Pell City, AL, employees were exposed to a fall of 10 feet while using an extension ladder that was not extended at least 3 feet above the landing of the roof.
