29 CFR 1926.451(b)(1)(i):   Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.    a) 1100 Albany Avenue - On or about - 2-4-13 - Employee installing asphalt shingle on the roofs eave was working from a ladder jack scaffold system where the ladder jack was not fully planked or secured from movement.

29 CFR 1926.451(b)(5)(i):     The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilivered end.    a)  1100 Albany Ave. Job Site - On or about 2-4-13 - Employee installing shingles at the eave of a roof was exposed to a 9 feet 6 inch fall hazard by planking extending more than 12 inches beyond there support.      b)  1100 Albany Ave. Job Site - On or about 2-4-13 - Employee installing roof shingles on the pitched roof section was working from a roof bracket scaffold with planking extending more than 12 inches beyond one of the bracket supports.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a)  1100 Albany Ave. Job Site - On or about 2-4-13 - Employee installing roof shingles from a ladder jack scaffold did not have a ladder to access work platform exposing employee to 9 feet 6 inches fall hazard to the ground.
