29 CFR 1910.305(b)(2)(i):     Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:    (a) On or about January 29, 2013 at The Westin Ka'anapali Ocean Resort Villas in the north chiller room a box on the back wall in the second room was missing a cover leaving the wiring exposed; and    (b) On or about January 29, 2013 at The Westin Ka'anapali Ocean Resort Villas in the north chiller room a box on the side wall in the second room was missing a cover leaving the wiring exposed; and    (c) On or about January 29, 2013 at The Westin Ka'anapali Ocean Resort Villas in the security office just inside the entry door an electrical cover was not attached to the junction box leaving the wiring exposed.
