29 CFR 1926.652(c)(2)(iii):     Manufacturer's specifications, recommendations, and limitations, and manufacturer's approval to deviate from the specifications, recommendations, and limitations shall be in written form at the jobsite during construction of the protective system. After that time this data may be stored off the jobsite, but a copy shall be made available to the Secretary upon request.    a.  At the construction site, located at Liberty Ave and Burnett Rd in Kanauga, Ohio, the employer did not maintain the manufacturers' specifications, tabulated data or a registered professional engineer design for the trench shield employee's were working in at the bottom of an approximately 22 feet deep trench.

29 CFR 1926.652(a)(1):     Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section.    a.  At the construction site, located at Liberty Ave and Burnett Rd in Kanauga, Ohio, employees were working in a trench that was approximately 22 feet deep, 6 feet wide and 42 feet long that did not have an adequately designed protective system thereby, exposing employees to a cave-in hazard.    Southern Ohio Trenching & Excavating, Inc., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 314100421, citation number 01, item number 6 and was affirmed as a final order on March 17, 2010, with respect to the workplace located at 415 Heplan Street, Ironton, Ohio 45638.

29 CFR 1926.652(g)(2):     Additional requirement for shield systems used in trench excavations. Excavations of earth material to a level not greater than 2 feet (.61 m) below the bottom of a shield shall be permitted, but only if the shield is designed to resist the forces calculated for the full depth of the trench, and there are no indications while the trench is open of a possible loss of soil from behind or below the bottom of the shield.    a.  At the construction site, located at Liberty Ave and Burnett Rd in Kanauga, Ohio, employees used a trench shield that had approximately 3 feet below the trench shield that did not have cave-in protection.     Southern Ohio Trenching & Excavating, Inc., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 314100421, citation number 01, item number 7 and was affirmed as a final order on March 17, 2010, with respect to the workplace located at 415 Heplan Street, Ironton, Ohio 45638.
