29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:       a.  235 Conway Drive, Bogart, GA (Digital Printer Room) - Employees were exposed to an entrapment hazard.  The designated, marked, and identified emergency exit door in the digital printing press room was blocked with a large metal cabinet.  The cabinet was stored directly in front of the emergency door.  In the event of emergency, employees would not be able to access the emergency exit door, on or about February 1, 2013.

The employer did not have machine specific written procedures for locking out/tagging out the multi-energy machines to safe guard employees performing services and/or maintenance on the printing presses, on or about February 1, 2013.       a. 29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:   a.  235 Conway Drive, Bogart, GA (Throughout Shop Area) - Authorized employees were  exposed to an unexpected energy release/start-up hazard. The employees were not applying energy-isolating devices such as locks to the electrical disconnect boxes when shutting down the power to perform repair and maintenance to the printing press machines, on or about February 1, 2013.               a.
