29 CFR 1910.23(a)(1): Every stairway floor opening shall be guarded by a standard railing constructed in accordance with paragraph (e) of this section. The railing shall be provided on all exposed sides (except at entrance to stairway). For infrequently used stairways where traffic across the opening prevents the use of fixed standard railing (as when located in aisle spaces, etc.), the guard shall consist of a hinged floor opening cover of standard strength and construction and removable standard railings on all exposed sides (except at entrance to stairway).    On or about 01/30/2013, the employer did not ensure that the stair opening in the loading dock was guarded to prevent employees from stepping into the opening. The stair opening, located at the mid-point of the loading dock face, did not have guardrails or safety chains installed along its edges. The opening was approximately 4 ft from the edge of the loading dock to the back of the truck ramp.

29 CFR 1910.215(a)(2): The safety guard shall cover the spindle end, nut, and flange projections. The safety guard shall be mounted so as to maintain proper alignment with the wheel, and the strength of the fastenings shall exceed the strength of the guard.    On or about 01/30/2013, the employer did not ensure that the "Tool Exchange" Model #106L+1B bench grinder, located adjacent to the overhead door on the south wall of the repair shop, was equipped with a side guard to cover the rotating shaft and other rotating parts attached to the shaft.  Loose cllothing/jewelry could get caught by the rotatiing parts of the grinder resulting in serious injuries.

29 CFR 1910.215(a)(4): On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.    On or about 01/30/2013, the employer did not ensure that the "Tool Exchange" bench grinder, Model #106L+1B, was equipped with a worik rest for both abrassive wheels. The work rests assist the employee in supporting the material being worked; without the work rest, the material being worked can be drawn into the abrassive wheel resulting in the work becoming a projectile and/or breaking the abrassive wheel.

29 CFR 1910.215(b)(9): Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.)    On or about 01/30/2013, the employer did not ensure that a opening where work is performed on the abrassive wheel on the "Tool Exchange" bench grinder, Model #106L+1B, was properly constructed to allow the tongue guard to be adjusted constantly.
