29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about February 4, 2013, an employee was exposed to fall hazards in excess of 25 feet, while installing a new roof without fall protection. Abatement Note: Abatement certification and documentation are required for this item. (see enclosed Certificate of Corrective Action worksheet)

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: (a) On or about February 4, 2013, framing employees were exposed to tripping and falling hazards while accessing the inside of the home under construction that was not provided with a ramp. The change in elevation from the ground level to the first floor landing that was in excess of 22 inches Abatement Note: Abatement certification is required for this item. (see enclosed Certificate of Corrective Action worksheet)
