29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:      a) On or about 02-05-2013 at the site, the lifting platform (pallet) installed on the forks of a JCB-532 Loadall forklift used to carry employee(s) to the roof was not authorized and/or secured exposing the employee to a 10 foot fall hazard.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    a) On or about 02-05-2013 on the rooftop (5/12 slope) where the roof was being repaired, replacing damaged tiles, no fall protection was enforce by the employer, exposing employee to a 10 foot fall hazard.
