29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      Facility:  The employer did not have documented procedures to describe how potentially hazardous energy would be addressed during servicing or maintenance of machinery or equipment.    On or about 04 February 2013 an employee was injured from a truck bed hooklift that returned to the flat position while raised during an inspection to attempt to identify a malfunction.  The employees arm was broken as a result of the bed rapidly falling back into place.
