29 CFR 1926.102(a)(3):     The vision of employees requiring use of corrective lenses in spectacles was not protected by goggles or spectacles of one of the types listed in paragraphs (i) through (iii) of this section:    A worker was observed cutting lumber with a reciprocating saw and using a pneumatic nail gun while only wearing corrective lenses.  The corrective lenses did not meet the requirements specified in American National Standards Institute, Z87.1-1968, Practice for Occupational and Educational Eye and Face Protection.

29 CFR 1926.501(b)(13):     Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    At the jobsite, workers were exposed to fall hazards while performing roofing activities on a low slope roof without the use of any fall protection system:            a) Workers were observed walking the top chord of the trusses and exposed to a 10 foot fall to the second story floor.          b) Workers were observed sheathing the roof deck and exposed to a 22 foot fall hazard to the ground below.    HP. Renovation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection number 315979898, Citation 1, Item 1, issued on 02/28/12, and became a Final Order on 03/23/12 with respect to a workplace located at 3606 Harmann Estates Drive, Bridgeton, MO, 63044.
