29 CFR 1926.21(b)(2):  The employer did not instruct each employee in the recognition and avoidance of  unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    For work performed on or about January 11, 2013, at a new building construction site:  Employees erecting steel beams approximately 43 feet above the ground were allowed to stand and work from the mid-rail of a scissor lift basket and the top rail of an employer constructed basket, exposing employees to the hazard of falls.
