29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  On or about February 5, 2013, at 275 Ridge Walk Court, Sugar Hill, Georgia:  Two employees installing a water meter were exposed to cave-in hazards when they worked inside an approximate 5.8-feet by 5-feet, 5.7 feet deep excavation that did not have a ladder installed for safe access and egress.
