29 CFR 1910.147(c)(4)(ii):     Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h):      A.)  Charlies Specialties, Inc. - On or about 2/13/13, employees performed maintenance and servicing work to equipment including, but not limited to, conveyors, cutting blades, and a kettle.  The lockout/tagout procedures for the machinery did not include instructions on how to drain stored energy, energy magnitudes, steps to verify deenergization, and steps to release the lockout.

Charlie's Specialties, Inc. - On or about 2/13/13, employees were operating forklifts without having a current evaluation performed of their abilities.  The last training and/or evaluation was performed in November 2009.
