Instance 1: Main floor - 2 extension cords by bread slicer in use that were missing ground pins.  Instance 2: Main floor - Cord on the strapping machine had pulled out of the grommet on the plug exposing conductors.  Instance 3: Main floor - Cord on bread slicer had pulled out of the grommet on the plug exposing conductors.  Instance 4: Main floor - Cord on the mixer had pulled away from the disconnect exposing conductors.  Instance 5: Main floor - Extension cord by bread slicer had damaged jacket exposing conductors.
