29 CFR 1910.67(c)(2)(v):     A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:      a) On or about 2/11/13, on jobsite at 1460 Military Rd.,Kenmore, NY: Two employees were exposed to potential fall of approx. 15 ft. while doing roofing work on the north side of the building from the "basket" of a "JLG" aerial lift. Neither employee was wearing a body harness/belt with a lanyard, nor were they tethered to the boom/basket in any other way.      NO ABATEMENT CERTIFICATION REQUIRED
