29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.                     a)  On or about February 6, 2013, employees were exposed to fall of greater than 6 feet while they walked and worked on a roof in order to install siding and perform clean-up without the use of adequate fall protection.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:      a)  On or about February 6, 2013, employees were exposed to falls of up to 10 feet while using a ladder which did not extend at least 3 feet above the upper landing.
