29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a)  At the site, on February 8, 2013, where employees were performing roofing activities without the use of fall protection.  The roof was flat and greater than 21 feet in height.  This condition exposed the employees to a fall hazard.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:    a)  At the site, on February 8, 2013, where employees used a Werner extension ladder to access a roof structure that was greater than fourteen feet in height.  The ladder was not extended at least three feet above the roof landing.  This condition exposed the employees to a fall hazard.

29 CFR 1926.1053(b)(4):  Ladder(s) were used for purposes other than the purpose for which they were designed:     a)  At the site, on February 8, 2013, where employees used a Werner step ladder to access a roof structure.  The ladder was in the closed position as the employees progressed from one roof landing to the next roof landing.  This condition exposed the employees to a fall hazard.
