29 CFR 1910.132(a):     Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:     On or about February 12, 2013, employees engaged in the cleaning of screens in the "reclaiming" area were not required to use goggles to protect from the hazards of cleaning chemicals, including, but not limited to, 2-Butoxyethanol and Propylene Glycol Ether, known to cause eye irritation and damage.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1904.29(a):     The employer did not use an OSHA 300 and 300A Form or equivalent.     On or about February 12, 2013, the employer forms used for recording injuries and illnesses were not an equivalent form for the calendar year 2012. A recordable injury that occurred on October 23, 2012 was neither recorded on the OSHA 300 log nor the 300A form, as the employer did not use either form.     Abatement certification and documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(c) and 29 CFR 1903.19(d), respectively.

29 CFR 1910.1200(e)(1):     The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:     On or about February 12, 2013, the employer did not develop, implement, and/or maintain at the workplace a written hazard communication program when employees were exposed to products such as screen printing inks and screen washes containing chemicals such as Propylene Glycol Ether and 2-Butoxyethanol among others, which pose skin and inhalation hazards.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
