29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:    a) At the Warehouse, on or about and at times prior to, April 30, 2013, employees connected a Porta-A-Cool evaporative cooling unit to a 110 volt electrical duplex receptacle outlet that did not have a faceplate, exposing employees to electrical shock and burn hazards.    ABATEMENT CERTIFICATION IS REQUIRED.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  a) At the Warehouse, on or about and at times prior to, April 30, 2013, the employer did not provide basic advisory information contained in Appendix D of the Respiratory Protection standard to employees who used filtering face piece respirators (dust masks) on a voluntary basis, exposing employees to respiratory and dermal hazards.   ABATEMENT CERTIFICATION IS REQUIRED.
