29 CFR 1910.37(b)(6):     Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color. Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m2) are permitted.    On or about 12 February 2013, and at times prior thereto, three exit signs within the main office building and one exit sign inside the warehouse were not illuminated, exposing employees to fire hazards.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure Exit signs are illuminated to meet the requirement.

29 CFR 1910.242(b):     Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.    On or about February 12, 2013, and at times prior thereto, employees used compressed air at pressures of approximately 42 psi for cleaning purposes.  The safety relief device, such as the reducer section, was not present on the tool to prevent the hazard of flying projectiles.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure safety relief devices are attached to the air nozzles of the compressed air guns to help reduce the pressure to 30 psi.

29 CFR 1910.303(g)(1):     Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    On or about 12 February 2013, and at times prior thereto, three (3) electrical 480 volt breaker panels all in different work areas were blocked and had materials stored in front, limiting safe and ready access, and exposing the employees to slip, trip, and fall hazards.;     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure the working space around electrical panels are kept clear up to 36 inches or more to allow safe and ready access.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail as required by the regulation.    For calendar year 2011, two cases were over-recorded. Entries 14 and 19 were marked in column H (Days away from work) and in column I (Job transfer or restriction) instead of just one. Only the most severe outcome shall be marked.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure the Log is completed correctly in all areas.

29 CFR 1904.32(b)(6):     At the end of each calendar year, the employer did not post the OSHA 300A Summary by February 1 as required by 1904.32(a)(1) and/or keep it posted for three months, until April 30th as required by 1904.32.    At the inspection site, for calendar year 2012, the OSHA Form 300A Summary of Work-Related Injuries and Illnesses was not posted by February 1st as required by 1904.32(a)(1).    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure the Summary is signed and posted between dates requested February 1st until April 30th.
