29 CFR 1926.150(c)(1)(vi):     A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:    (a) On Febuary 12, 2013 at 110 Cunningham Parkway Belton Missouri 64012. Employees were exposed to a fire hazard by storing a 5 gallon gas container on the scaffold  without an approved fire extingisher within 50 feet.

29 CFR 1926.451(g)(1):     Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:      (a) On Febuary 12,2013 at 110 Cunningham Parkway Belton,Missouri, employees were exposed to a fall hazard while working on a supported frame scaffold over 10 feet from the ground without guardrails or proper fall protection.
