(a) On or about 02/13/2013 at 74 Lincoln Avenue, Carteret NJ 07008: Employee was engaged in siding activities were exposed to fall hazards of approximately 18' from the scaffold when an inspection of the scaffold system for visible defects was not provided.

(a) On or about 02/13/2013 at 74 Lincoln Avenue, Carteret NJ 07008: Employee was exposed to fall hazards of approximately 18' from a pump jack scaffold platform in which a fall protection system was not erected or used by employee engaged in siding construction.
