29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition.  Sources of ingition may include open flames, lightning, smoking, hot sirfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat:    (a) Brine Water Holding Tanks:  Employees use Streamlight Part #61400 Endro LED headlamps for illumination during hours of darkness when checking brine tank levels.  Employees open hatches/vents to take tank level readings while wearing these headlamps that are not intrinsically safe, introducing a potential source of ignition where flammable vapors may be present, as determined on March 5, 2013.    Abatement Certification Required for this Item.

29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:    (a) Mechanic's Garage:  The employer provides portable fire extinguishers throughout the mechanic's garage.  The employer had not provided all employees with an educational program to familiarize them with general principles of the fire extinguisher use and the hazards involved with fighting an incipient stage fire upon initial hire and annually thereafter, as determined on March 6, 2013.
