29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.    a)  At 10001 Summer Wood Court Mobile, Alabama:  On or about February 8, 2013 and at times prior; the employer failed to ensure employees using a pneumatic nail gun to install shingles were wearing eye protection exposing them to struck-by hazards.

29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)  At 10001 Summer Wood Court Mobile, Alabama:  On or about February 8, 2013 and at times prior; the employer failed to ensure employees installing shingles on a 7:12 pitch roof had fall protection in place to prevent them from falls of 9.3 feet to the ground below.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) At 10001 Summer Wood Ct Mobile, Alabama:  On or about February 8, 2013 and at times prior; the employer exposed his employees to fall hazards of 8.9 feet to the ground below in that he failed to ensure an extension ladder used to gain access to the roof extended at least three feet above the landing area.
