29 CFR 1910.157(g)(1): Where the employer had provided portable fire extinguishers for employee use in the workplace, the employer did not also provide an educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting: a) Job site, 2007 Sheffield Road, Aliquippa, PA: On or about February 14, 2013, during pharmacy activities, the employer provided fire extinguishers and expected employees to use them, but did not provide any fire extinguisher training or instruction.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative: a) Job site, 2007 Sheffield Road, Aliquippa, PA: On or about April 16, 2013, during activities such as jam clearing or label reloading activities on the Parata pill dispenser unit, the employer did not ensure that an appropriate energy control program was in place and being used, exposing workers to the hazard of being caught on/in the machinery's moving parts.
