29 CFR 1926.800(o)(1): Portal openings and access areas in were not guarded by shoring, fencing, head walls, shotcreting or other equivalent protection to ensure safe access of employees and equipment: a) Worksite, 1316-1330 Willow Ave., Hoboken, NJ The employer directed employees to access an area of soil removal approximately 14 feet below the first concrete slab through an opening in the concrete via a temporary ladder. There was a pile of soil approximately 7 feet from this ladder, approximately 12 feet high , 10 feet wide, and 6 feet wide, with a pitch of appproximately 70 degrees. The employees who descended and ascended this ladder were not protected from being struck by this wall of soil in the event of collapse. Violation occurred 2-7-13
