29 CFR 1910.147(c)(5)(ii): Lockout devices and tagout devices shall be singularly identified; shall be the only devices(s) used for controlling energy; shall not be used for other purposes; and shall meet requirements A thru D:    On February 19, 2013, at the job site located at 3275 Tierra Blanca Rd, Hereford, Texas 79045, the lockout/tagout devices did not meet the requirements of the standard for shutting down, isolating, blocking and securing machines and equipment containing hazardous energy, including but not limited to:  mixer #1 and mixer #2.  Employees were not assured of full protection against such hazards as being caught-by moving equipment or electrical shock. 29 CFR 1910.147(c)(7)(i):   Employee training did not include all of the elements listed under items (a) through (c) of this section;       On February 19, 2013, at the job site located at 3275 Tierra Blanca Rd, Hereford, Texas 79045, the training provided for employees was inadequate, in that, training did not include the use of specific procedural steps for locking out or tagging out of machines/equipment.  Employees were exposed to hazards associated with the unexpected energization, or start-up of machines/equipment, or the release of stored energy.
