29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:    a)  Imperial Ave & Imperial Blvd, Weehawken, NJ.    Employee climbing out of an aerial lift basket onto balcony platform, thereby exposing the employee to a fall hazard of 40 feet to the ground below.                                       Violation occurred on or about 02/12/2013.    b) Employee was observed coming back into the aerial lift basket, thereby exposing the employee to fall hazard of 40 feet to the ground below.                                      Violation observed on or about 02/12/2013.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a) Imperial Ave & Imperial Blvd, Weehawken, NJ.    Employees were observed working out of an aerial lift basket without the use of their harness/body belt, thereby exposing the employees to being thrown out of the aerial lift basket while being occupied.                                       Violation observed on or about 02/12/2013.
