29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:  On or about February 14, 2013, on the north side of the Toolpusher's trailer, where two 220V nominal power leads on the transformer were pulled from the packing gland. Taped repairs did not duplicate the cord's original characteristics; neither the jacket's strength nor flexibility characteristics were restored. The depth of the abrasions and cuts could not be monitored to see if they would get worse without removing the tape.
