29 CFR 1926.451(f)(1):  Scaffold and/or scaffold components were loaded in excess of their maximum intended loads or rated capacities, whichever was less:    a)  At the site, on February 20, 2013, where employees stood on the guardrailing system of the Sunbelt scissor lift, model #GS-2668RT to access and egress from the roof structure.  The guardrail system was not designed to receive an imposed load from employees standing on the railing.  This condition exposed the employees to a fall hazard.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a)  At the site, on February 20, 2013, where employees were covering a newly constructed portion of a building fascia with a plastic tarp while standing on a roof structure.  There was no fall protection system installed to protect the employees from falling to the ground level.  This condition exposed the employees to a fall hazard.
