29 CFR 1926.760(a)(1):   Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    a. On or about 11/14/2012, Sergio Alcorta, P.E., Estate Tutu Lima- Fire Station, # 390 & 386-1 Annas Retreat, St Thomas V.I., Garage roof area: Employees engaged in steel roofing operations were observed at the edge of a roof, with an unprotected side or edge at 19 ft high from the lower level without a conventional fall protection system, such as guardrail, safety net, or personal fall arrest system in place.      NOTE:    Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

On or about 11/14/2012, Sergio Alcorta, P.E., Estate Tutu Lima- Fire Station, # 390 & 386-1 Annas Retreat, St Thomas V.I., Garage roof area: The employer did not train each employee exposed to a fall hazards. Employees were working in steel roofing operations at the edge of a roof, with an unprotected side or edge at 19 ft high from the lower level without a conventional fall protection system.     NOTE:    Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

On or about 11/14/2012, Sergio Alcorta, P.E., Estate Tutu Lima- Fire Station, # 390 & 386-1 Annas Retreat, St Thomas V.I., Garage roof area: The employer did not train each employee exposed to a fall hazards while using ladders.  Employees were using a telescopic multipurpose ladder to gain access to steel structure roof area at 19 ft high from the lower level.    NOTE:    Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

On or about 11/14/2012, Sergio Alcorta, P.E., Estate Tutu Lima- Fire Station, # 390 & 386-1  Annas Retreat, St Thomas  V.I., Garage roof area:   Employees were using a telescopic multipurpose ladder not extending at least 3 feet above the upper landing surface to which the ladder was used to gain access to steel structure roof area.      NOTE:  Certification and documentation of abatement must be submitted for this citation item as required by 29. CFR 1903.19(c) and (d), respectively.
