29 CFR 1910.305(b)(2)(i):  All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    Twine Shop - One receptacle box cover was broken    Downeast Fishing Gear, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 314540352, Citation number 1, Item number 5 and was affirmed as a final order on (10/13/2010), with respect to a workplace located at 12 Bar Harbor Road, Ellsworth, ME.

Twine Shop - Conductive duct tape was covering the two 110 V and one 220 V energized receptacles.  Duct tape is not approved for use on electrical equipment.

29 CFR 1910.303(g)(1)(i)(A):   For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:    Warehouse and Twine Shop - The three breaker panels did not have 36" clearance in front to allow access.
