29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Mill Area: On or about 14 February 2013 and at times prior the flour packaging station was being run with one of the equipment cage doors removed, thereby exposing employees to machine contact during the filling and settling operating cycle.

The chop saw power cord had been wrapped with electrical tape in two areas where the outer sheath had been damaged.
