29 CFR 1926.251(a)(6): Each day before being used, and during use, where service conditions warrant, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person, and damaged or defective slings were not immediately removed from service:    A.	 On or about 02/21/13, at the site of 238-01 Horace Harding. Douglaston, NY    Employees using slings to unloading a switch board panel approximately 2000lb. The sling was worn and had an oily residue.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

29 CFR 1926.1425(c)(1): The employer did not make sure that the materials being hoisted when employees were hooking, unhooking, or guiding the load were rigged to prevent unintentional displacement.    A. On or about 02/21/13, at the site of 238-01 Horace Harding. Douglaston, NY    Employees using slings to unloading a switch board panel approximately 2000lb. from a truck. The panel fell and struck an employee.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

A hook on the boom truck was used without self-closing lactches.  ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19
